The model and photographer are unknown.
The mural is a street art piece by Herakut. They are a two-person team of German street artists specializing in murals. Jasmin Siddiqui, or "Hera," and Falk Lehman, "Akut".
It was created April 2011 or earlier, in Los Angeles, USA. I don't believe the piece has a name, as I could only find two other walls in Los Angeles and this piece was not among them.
